Support For Independent Living

Program Description:

  • Assisting  individuals with serious mental illness to live as independently as possible in the community
  • Program offers in-home case management support to registered clients
  • The focus is on persons in high need of support
  • Services are provided 7 days a week with 24 hour emergency assistance
  • Geographical service area: North Simcoe (Midland-Penetang area)
  • The program strives to ensure that its clients are satisfied and successful where they live, learn, work, and/or socialize
  • Program uses the psychiatric rehabilitation model (developed at Boston University) which is a best practice
  • The case managers work with their clients to develop and implement a rehabilitation plan which focuses on goal setting, planning, skill development, and establishing sufficient supports to help them achieve their goals

Our Team:

  • We are a team of full-time mental health workers
  • Employees of this program have backgrounds in the areas of psychology, sociology, criminal justice, developmental services, addictions, nursing, and social service work. 

 

Eligibility Criteria:

  • Adult mental health consumers (age 16+) with a serious mental illness (such as bipolar disorder, borderline personality disorder, schizophrenia, and other psychosis)
  • Reside within North Simcoe (Midland-Penetang area)

 

Hours of Service:

  • Program operates 7 days a week, including weekends and statutory holidays
  • Hours of service are primarily 9am-9pm on weekdays and 11am-7pm on weekends/statutory holidays
  • Program offers 24 hour emergency support to its registered clients

  • Individuals can self-refer, or family members, doctors, other agencies, hospitals, etc. can refer
